These roles will provide clinical, analytical, digital and programme support to help
translate population health insight into practical, local action - working with our 14 PCNs and 81 practices and partners to improve outcomes, access and equity for our communities.

Role profile
A strategic clinical leadership role focused on improving how neighbourhoods respond to people living with frailty, complex and multiple needs, and those approaching the end of life. The postholder will work across organisational and professional boundaries to support pathway redesign, clinical stewardship, population-health-informed action, quality improvement and stronger integrated working.
It is one of nine countywide portfolio lead roles in PCNA’s clinical leadership model, funded through NHS Lincolnshire ICB’s clinical leadership and neighbourhood development allocation. Frailty and complex care is a lead priority for Integrated Neighbourhood Team (INT) development in Lincolnshire.
