Working Together for a Healthier Lincolnshire

Lincolnshire Primary Care Network Alliance (LPCNA) is a collaborative organisation dedicated to supporting Primary Care Networks (PCNs) and GP practices across Lincolnshire. We work to strengthen primary care, drive innovation, and foster partnerships across health and social care to improve patient outcomes.

Our Purpose

The Lincolnshire PCN Alliance exists to support and strengthen Primary Care Networks and General Practice through collaboration, leadership, and effective system partnership.

The Board will work collectively to:

  • Increase General Practiceresilience and stability for local populations.
  • Act as the collective vehicle for Population Health Management to improve the healthcare of local communities.
  • Bring about collaboration between PCNs on behalf of General Practice.
  • Create opportunities for PCNs to work at scale outside the Network Contract Directed Enhanced Service (DES) framework.
  • Serve as the unified voice for PCNs, influencing system outcomes on behalf of General Practice.

Our Vision

The Lincolnshire PCN Alliance seeks to build a sustainable, equitable, and thriving model of Primary Care in Lincolnshire.

The Board’s vision is to:

  • Create opportunities for PCNs to achieve their objectives and thrive.
  • Work alongside system partners to build General Practice resilience.
  • Act as an equal partner within the wider healthcare system.
  • Respect the autonomy of individual PCNs and be aware of local population health challenges and determinants.
  • Help ensure that different forms of resource are identified and shared to optimise delivery for population health in Lincolnshire.
